January 2021 Special Edition
Two changes will be pushed into the system on Monday, January 18, 2021.
First: New Eviction questions will appear on the Universal Data Assessment during the entry assessment workflow.
The purpose of these new questions is to determine if eviction is a direct cause of homelessness, and if so, how much of an effect does eviction rates have on people seeking homeless services. We will use this information in conjunction with eviction counts in Utah to determine how severe eviction affects those seeking services.
In the last 12 months, have you been evicted?
Are you homeless today because of an eviction?
Have you been denied housing due to an eviction?
The questions may be responded to as:
- Data not collected
- Client Refused
- Client doesn’t know
None of the questions are required to save the form and are not expected to be collected for any client whose enrollment opened before 1/18/21.
We were also able to fix the programming for the prior zip code. The Prior Zip Code question will only appear during the entry assessment and only for heads of households and other adults associated with the case.
Second: Comment box in Coordinated Entry Triage Assessment
A comment box has been added to the triage assessment used during the Coordinated Entry intake box. No changes to processes need to occur at this time. This box was added to help communities expedite their coordinated entry processes to quickly identify where clients are within their By Name List processes.
Comments are restricted to 100 characters and will affect how the By Name List is displayed. Comments are overwritten every time it is changed and is not meant to be a tracking record for CE, the other services programs, and items entered into the system will serve as tracking for reports.
UHMIS Use Tip
Document Check (uploading files)
We first discussed the new document check feature in the November 2020 UHMIS newsletter, and now we have an exciting addition to it. You may now upload documents directly into UHMIS.
As a reminder, The Document Check feature was pushed into the system when Coordinated Entry was started. It is a flexible tool that can track all sorts of documents, like identification, income verification, job applications, and UHMIS Informed Consent Release forms.
Remember, only store documents directly in ClientTrack if they agree to their information shared this way across all accessing agencies.
1. Navigate to Document Check
2. Select ‘Add New Document Check’
3. Enter the information for the document you are uploading. In this case, we are adding a UHMIS Informed Consent Release form, but you could also add an ID, SSC, or any number of other documents.
4. Select ‘Choose File’ to bring up your file explorer and find the file you wish to upload. Please keep PPI out of the document name (e.g., name documents 456789 - Informed consent, instead of Jane Doe - Informed Consent)
