SD Department of Health has recommendations for completing your online Assisted Living Center license application.
· Section II.D.10 is a new section for Secured/Memory units. Please review carefully as we have approximately ten memory units in ALCs at this time. Just because you are licensed for cognitive impairment does not necessarily mean you have a secured unit.
· Section III.B. Governing Body provide only the President, VP, Secretary, and Treasurer, their respective address, and their board position. The phone number is only required for the President of the GB.
· Section III.C. Staffing, the required fields are for a licensed nurse, UAP supervisor, pharmacist, RD (If you provide therapeutic diets), and the certified dietary manager(CDM)/person-in-charge of kitchen. For the CDM/person-in-charge of kitchen please use either the CDM certificate number or the individual’s ServSafe certificate number and expiration date. Instances where the licensed nurse and the UAP supervisor are the same individual use a second drop down box to enter the required field for licensed nurse AND UAP supervisor. Ensure you have the current license information for the licensed nurse, pharmacist, and dietitian. Remember all RD’s licenses expire 6/30/16. If you submit your license after 6/30/17 you will need to have your RD’s current license on file for 6/30/17. All pharmacist’s licenses expire 9/30/16.
· Do not provide any personal information such as addresses for the licensed nurse, pharmacist, RD or CDM/person-in-charge of kitchen other than license number or certificate number. That information is only required for the Governing Body (section B) or the Board of Directors (section F) if those directors are different from section B (Governing Body).
