Purkinje Cell Mosaic 1
Euroimmun's new Purkinje Cell Mosaic 1 allows the determination of autoantibodies Yo/CDR2, DNER, ITPR1, and CARP VIII. 
Along with Multiple Sclerosis and Acute Disseminated Encephalomyelitis, autoantibody-associated disorders of the CNS are the most common cause of autoimmune cerebellar ataxia (ACA).

While ACA is a rare manifestation in some disorders (such as in Aquaporin-4 associated Neuromyelitis optica), it is the most prevalent or exclusive presentation in others.  To date, around 30 different autoantibody targeting brain antigens have been described in patients with ACA - many of which are of paraneoplastic origin.  

Paraneoplastic cerebellar degeneration (PCD) is a common cause of subacute ACA, caused by widespread loss of the Purkinje cells of the cerebellum.  PCD diagnosis can be confirmed by detection of onconeural antibodies that may also indicate the underlying tumour type.  Most onconeural antibodies associated with PCD recognise intracellular antigens in Purkinje cells. 
This indirect immunofluorescence mosaic test allows detection of the well-characterised autoantibodies to Yo/CDR2 and DNER/Tr, and allows the determination of the lesser-known autoantibodies ITPR1 (inositol 1,4,5-trisphosphate receptor type 1) and CARP VIII (carbonic anhydrase-related protein VIII).  Typically, these autoantibodies bind to the axons, dendrites, and the somata of Purkinje cells.  

The Purkinje Cell Mosaic 1 is a recombinant cell-based indirect immunofluorescence test, and is composed of the following BIOCHIPs in order to enable the monospecific determination of autoantibodies:
  • Yo/CDR2 (Acetone fixed) - Cut-off 1:100
  • DNER (Formalin fixed) - Cut-off 1:10
  • ITPR1 (Acetone fixed) - Cut-off 1:100
  • CARP VIII (Acetone fixed) - Cut-off 1:10  
Currently the DNER BIOCHIP is CE-marked, and the remaining substrates are intended to receive CE-marking soon.
